The app provides education, self-tracking and clinic-contact tools. It does not itself create a treatment agreement, appointment, prescription or guarantee of eligibility or outcome.
Responsible use
Use the app only for lawful purposes and for information you are entitled to provide. Do not share another person's health information without authority or attempt unauthorized access.
Availability
Features may change or be temporarily unavailable. Mandatory patient, consumer and data-protection rights are not excluded by these terms.
